They can be found on Facebook as "FJK Medical Services LTO Accredited" They have a phone number on their facebook page. 400PHP for Medical Certificate in person. Appointment not needed, but recommended. If you are applying for Medical Certificate, go in the morning AM times - the doctors leave in the afternoon.
